1. Download the application form (Annexure-I) from the official website of Deendayal Port Authority.
2. Fill it carefully in block letters and paste a recent passport-size photograph on the top right corner.
3. Attach self-attested copies of your 7th pass certificate, experience certificate, photo ID proof (Aadhaar, PAN or Voter ID) and caste certificate, if applicable.
